3. Date and sign the printed form.
4. Enclose $10.00 (NON-REFUNDABLE) money order, NO CHECKS filing fee for each vehicle permit requested.
5. Attach a photocopy of the vehicle registration.
6. Mail application, vehicle registration, and filing fee to:
